The Continuing Care Hospital (CCH) is licensed as a general acute care
hospital, and addresses the unique and complicated physical, emotional,
spiritual, nursing, and rehabilitation needs of the long-term acute care
patient.
CCH has a seperate and dependent governing board with its own Medical
Directors.
There is no limitation regarding diagnosis type, patient age, length-of-stay,
and scope of services provided.
CCH Serves patients who have:
- Complex Medical Conditions
- Respiratory Infections and Disorders
- Ventilator Dependance
- Long-term IV Antibiotic Therapy
- Cardiac Conditions
- Wound Care Needs
- Complex Post-surgical Conditions
- General Debilitation
- Rehabilitation Capability with Complicated Needs
- Length-of-stay Exceeding 25 Days
What is a long-term acute care hospital?
It is a special classification of hospital recognized by the federal
government for facilities that meet required specifications, including
maintenance of an average length of stay of at least 25 days per patient and
meeting all conditions of participation as an acute care hospital with regard to
administration and basic hospital functions.
CCH acts as a "hospital within a hospital", leasing its space as a tenant within Saint Joseph
Hospital and Saint Joseph East and scheduled to open in Saint Joseph Berea in Spring,
2007. As a separate and distinct entity, CCH provides a highly focused
environment of care for the needs of our patients.
Case Management at CCH:
- serves as the communication link between you, your
family, your physician and the staff.
- works with you and the interdisciplinary team to
enhance your care and your overall satisfaction while in the hospital.
- identifies obstacles and conflicts with your care
delivery and resolves them with the interdisciplinary team.
- coordinates care plan meetings to outline your
specific care needs and your progress towards discharge.
- facilitates and attains insurance approval and
certification for your inpatient stay at CCH.
- helps coordinate you hospital length-of-stay with
your physician and insurer.
- coordinates transfer to other facilities as needed or
coordinates your discharge to your home.
- coordinates Home Care Services and necessary medical
equipment.
