PGY1 & PGY2 Application

Applicants to the PGY1 and PGY2 Residency Programs at SJH are required to provide the following:
- Personal Cover Letter
- Curriculum Vitae
- Official College Transcripts
- Three Letters of Recommendation submitted with the Residency Applicant Recommendation Request Forms
- Completed Application
There are two acceptable methods for interested resident candidates to apply for either the PGY1 Pharmacy Residency or the PGY2 Cardiology Residency Programs:
1. Use ASHP’s Pharmacy Online Residency Centralized Application Service (PhORCAS)
Instructions for submitting application materials by this method:

Applicants choosing to apply through PhORCAS must download the Saint Joseph Hospital Residency Program Application.

Applicants can then upload their completed Saint Joseph Hospital Residency Program Application to the PhORCAS website along with all other materials required by PhORCAS.
Note: The Residency Program Application form may be used to apply for either the PGY1 Residency or PGY2 Cardiology Residency Program.

2. Apply directly to the Saint Joseph Hospital Residency Program without using the PhORCAS

- Email the following items to the appropriate Residency Program Director:
- Personal Cover Letter
- Curriculum Vitae
- Completed Saint Joseph Hospital Residency Program Application
- Download the Recommendation Cover Form and provide it electronically to your three professional references. The Recommendation Cover Form document provides instructions for how references should submit their letters of recommendation.
- Mail hard copies of your Official College Transcripts to the appropriate Residency Program Director.
All application materials must be received by the application deadline: January 6th.
